The study reveals a well defined enhancing soft tissue lesion in the region of vidian (pterygoid) canal on right

side.There is destruction of the walls of the sphenoid sinus adjacent to it. Anteriorly the lesion is extending upto the pterygopalatine fossa and posteriorly upto foramen lacerum. The lesion measures about 2.1 (anteroposterior) x 2 (craniocaudal)x 2(mediolateral)cms.

The lesion appears iso to hyperintense in T1 weighted images and heterogenously hypointense on T2 weighted images.

Foramen rotundum and ovale are normal. These is mass effect over roof of nasopharynx on right side. These is evidence of subacute infarction with hemorrhagic transformation in right middle cerebral artery Territory. There is absent flow in right internal carotid artery from its origin. Right anterior cerebral artery and middle cerebral arteries are filling from the contralateral side. Cavernous sinus appears normal.

There is no abnormal meningeal thickening noled in the brain.

Sinusitis involving bilateral frontal, ethmodal sinuses. Ill defined nodular enhancing lesion noted in right basifrontal location With mid surrounding edema.

Gliottie changes noted in right anterior temporal lobe. Subacute haemorrhagic infarct in right capsuloganglionic region.

Subacute infaret also in right parietal lobe.

Occlusion of the iternal carotid artery noted with prominent enhancement of right cavernous sinus.

To consider the possibilities

Granulomatous lesion right frontal lobe with right ica iocclusion with sinusitis (? Fungal) Subacute infarcts right capsuloganglionioc region.

vidian (pterygoid) . sphenoid .

lacerum pterygopalatine fossa posteriorly .
