Wound Care and Dressing

Procedure 1. Check the doctors order for specific wound care and medication instruction. 2. Wash your hands thoroughly. 3. Assess existing dressing. 4. Explain the procedure to the patient. 5. Secure the equipment and bring them to the bedside. 6. Loose and remove the dressing with the use of dressing forceps. If the dressing adheres to the wound, loosen it by moistening with sterile NSS from the center to the side in circular motion. 7. Observe dressing for the amount, type, color and odor of the drainage. 8. Discard the dressing in the waste receptacle. 9. Clean the wound aseptically using the dressing forceps from the center going outward in a circular motion using the following: a. Cotton balls with betadine cleanser b. Dry gauze(patting only) c. Cotton balls with betadine aseptic solution (use each gauze for only one stroke) 10. Apply new dressing by gently placing the gauze sponges at the wound center and moving progressively outward to the edges of the wound sites. 11. Secure the edges of the dressing to the patients skin with strips of adhesive tapes. 12. Make sure the patient feel comfortable and tidy the unit. 13. Do the aftercare of the equipment. Soak the dressing forceps in 5% Lysol solution for 30minutes, and then wash them with soap and water, rinse, them then dry. Send them to the CSR for sterilization. Remarks Done Not Done
