o Surveys for Continued Funding and New Project Allocations: The surveys on the
use of funds under Title II, Part B must be completed annually by state MSP directors
in order to provide the US Department of Education with a general picture of specific
priorities of RFPs, project start and end dates, duration of grants, and an overview of
any evaluation and accountability plans implemented.

o Copy of Newly Funded Projects: Each state director is expected to submit a copy of
each proposal selected for funding. If a project has received continued funding, an
additional copy of the proposal is not required.

o Listing of Discontinued Projects: In order to provide statistical data to federal


officials, each state director is asked to submit a listing of discontinued projects and a
brief narrative detailing why the project has not received additional funding.

o Annual Project Report (Project Profile, Project Narrative, External Evaluation):


According to the guidelines of Title II, Part B of No Child Left Behind, each project is
expected to submit an Annual Project Report demonstrating their progress towards
achieving the goals specified in their proposal. This report should include the Project
Profile, Project Narrative, and an External Evaluation. If a project has been
discontinued, the project should submit a Final Project Report in lieu of an Annual
Project Report.

Note: State MSP directors will receive all information prior to distribution to individually funded projects.
