You are hereby given notice that you are in breach of your tenancy of the premises located at 143 ilove you street unit-A , a 2 storey apartment, under the terms of the lease dated March 03 , 2011, between Pero Labo as landlord and Jos Linaw as tenant. You are in breach because you are 16 months delinquent in your payment to the undersigned of the sum of P 300,000.00 from the month of June 01 , 2011 to September 01 ,2012. In addition, you have failed to comply with the terms and conditions of your tenancy, as follows: Failure to pay P 20,000.00 as monthly rental since June 01 , 2011 despite several demands in oral and written form Failure to vacate the premises of unit-A of said apartment upon demand and written notice Failure to maintain in good condition of the facilities of the apartment within Unit A during the lifetime of the lease contract To remedy the above breach, you must do the following: To pay the unpaid rentals from June 01 ,2011 to September 01 2012 with an aggregate amount of P300,000.00 immediately with interest at 10% per month of unpaid rental To vacate the premises of Unit-A apartment situated at 143 Ilove you street , Laoag City within 10 days without extension.
