I hereby declare that to my knowledge the certificate of registration of the vehicle has not been either suspended or cancelled under any of the provisions of the Motor Vehicle Act , 1939 or rules made there under and herewith deposit (the fee of rupees Ten ) and apply for the issue of a duplicate certificate of taxation.
