Single Religion: Roman Catholic Phycisian: Dr. Itchon Chief Complaint: Left leg swelling Diagnosis: To consider Left Leg Cellulitis History of illness : The patient jump off a flatform, causing him to hit his left leg on the edge, he does not consulted any physician and numerous manipulation were done. Upon admission, there are pain and tenderness on the Left leg. Immunization: BCG, Hepa B. DPT No known Allergies. Physical Examination: The patient is conscious and irritated with the Vital Signs of: Temp. 36.2C RR- and 42bpn and PR-128bpm with long nails, untidy face and cast on the left leg. With Serous Discharge and capillary Refill of less than 3 seconds. Scabs and Scars were found at the right leg, which were caused by a blood infection as verbalized by the mother of the Patient.

Laboratory Test: Ultra Sound- (06/14/13)soft tissue swelling in the anterior aspect of the leg and extend it to the left knee, Impression: Consistent Cellulitis (06/24/13) presence of 14 x 4. 1 cm complex mass at anterior aspect of the leg . Impression: Pyomylitis

Hematology: Leukocyte Count 14.89 , Eosinophils- 0.11 Surgical Procedure: Debribment and Corticomy
