Therapeutic Category Antibiotic Pharmacologic class: Fluoroquinolone Use\ Indications Complicated intra-abdominal infection; Infectious diarrhea; Chronic bacterial prostatitis Mild to moderate, severe or complicated bone or joint, respiratory tract infection, skin or skinstructure infections Mild to moderate, severe or complicated UTI or pyelonephritis, Uncomplicated UTI Nosocomial pneumonia, Mild to moderate acute sinusitis Empirical therapy in febrile neutropenia Inhalation anthrax (postexposure) Anthrax prophylaxis, Plague, Travelers diarrhea, Tularemia Pharmacological Effects Inhibits bacterial DNA gyrase and prevents DNA replication, transcription, repair, and recombination in susceptible bacteria; bactericidal. Adverse Reaction CNS: seizures, confusion, headache, restlessness. GI: pseudomembranous colitis, diarrhea, nausea, vomiting. Hematologic: leukopenia, neutropenia, thrombocytopenia, eosinophilia. Musculoskeletal: tendon rupture. Skin: rash, Stevens-Johnson syndrome, toxic epidermal necrolysis. Precautions/Warnings/Contraindication Drug is associated with increased risk of tendinitis and tendon rupture, especially in patients older than age 60 and those with heart, kidney, or lung transplants. Use cautiously in patients with CNS disorders, such as severe cerebral arteriosclerosis or seizure disorders, and in those at risk for seizures. Drug may cause CNS stimulation. Pregnancy category: C Breast-feeding: Use cautiously Interactions: Cyclosporine: May increase risk for cyclosporine toxicity. Iron salts: May decrease absorption of levofloxacin. Separate doses by at least 2 hours. NSAIDs: May increase CNS stimulation. Monitor patient for seizure activity. Steroids: May increaserisk of tendinitis and tendon rupture. Monitor patient for tendon pain or inflammation Tizanidine: Increases tizanidine levels; low blood pressure, dizziness, and slowed psychomotor skills. Dosing Adult: 500 mg P.O. or 400 mg I.V. every 12 hours Children (1-17 years): 10 mg/kg I.V. or 15 mg/kg P.O. B.I.D ; Max: 800 mg/day I.V. or 1,000 mg/day P.O.
