    Prescribed Performa (Downloadable) Lady who is to undergo to tubectomy /laprosterilization has to come fasting, fill up the necessary form to be signed by husband and wife both and get blood or urine to be tested before actual surgery. Male coming for vasectomy need not come fasting and fill the form and get blood or urine to be tested before actual surgery. Place - General Hospital Sector -16 CHD Room No 147, Civil Hospital Manimajra Room No 12 Days - Monday to Saturday Time - 8 a.m to 2 p.m. Every patient who undergoes family welfare procedure is issued sterilization certificate. Government employees undergoing sterilizations gets benefits of 14 days leave in case of female and 6 days leave in case of male (undergoing vasectomy)
