On 21 August 2011 at 00:00 I was admitted to the Royal London Hospital. The reason was the adoption of Open Distal Tibia Fracture Right. August 23 I had surgery, intramedullary nail Tibia. The hospital was released 01 September 2011 at 15:49 I am writing this letter because of that, during my stay, and hospital discharge occurred as a result of a series of failures which have serious health defects. Please help clarify the issues that affect my stay at the Royal London Hospital. Open Tibia and Fibula fracture experienced as a result of the brutal attack on me. A man known by name to the police, armed with special shoes with soles and toes reinforced with steel sheet. He kicked me in the back of my right calf. Kicking foot armed with a shoe, caused an open fracture. This event took place on Sunday August 21, 2011. at 10:00 on Allen Garden at Code Street nearby Brick Lane. Forwards jeered at me until tonight. I do not know what was with me because I lost consciousness several times. At night the policeman found me and called an ambulance. I was homeless at the time of the event. But I get a benefit JSA and I had worked enough time in the UK to have the right to medical care and social care. In addition, I was victim, I was brutally attacked in the UK, therefore, had a right to treatment and care. At the hospital, I was granted a Social Worker. Her job was to take care of my needs and arranging temporary accommodation I live until a cure. The Social Worker has not fulfilled its obligatory. My social worker told me that they have a problem with me and they do not know what to do with me. She suggested that I should leave the hospital. She said, they will transport me to White Chapel Tube Station. Then someone finds me with an Out Reach Team and organize my temporary accommodation in the hostel. I refused, saying the White Chapel Tube Station is near White Chapel Mission where they gather bandits, and he will find me and kill me, before someone coming from the Outreach Team. I said you have no right to expose me to danger. After nine days of the operation came to my bed a man with a pair of scissors and cut the bandage on my leg. On the leg was a long scar and a lot of blue plastic surgical sutures and blood mixed with a transparent liquid flow from the wound. I do not assumed any other dressing, no wound disinfected. He said that I'm going to release from the hospital and needs no dressing. I was terrified, because it was major surgery not only skin and meat were cut but bone too. Inside bone was rod. It is very easy in this situation for infections that can attack the bone marrow. In the best case I risk amputation in the worst death. My social worker came to get me ready for discharge. I begged for help, would not deprive me of the street because they will kill me or I could die because of postoperative wound infection. She reassured me, saying to me that they organize for me temporary accommodation that driver will drive me to the Hounslow Civic Centre where they will take me to safe place. My social worker prepared the necessary documents that I had to give in Hounslow Civic Centre.

Day of discharge from the hospital the Royal London Hospital at White Chapel Road. In this discharge is written: Good postoperative recovery. Social input. It's all not true. I was discharged in very poor condition. It was a few days after the operation. At hospital discharge date is 1 September 2011year/ hours. 15:49. this is a wrong date because on that day I spent in the waiting room until 22:00 hrs. I sat on the Iron uncomfortable trolley to transport the patients to the short distance I sat on it for many hours for many hours without the possibility of changing the position. The seat of a leg down on a hard chair of the oppressed pulsating blood vessels causing unbearable pain and leg swells up like a balloon. No one arrived for me. On the eve of this excerpt was taken off my feet with bandages saying it would be better. Leg was without any dressing on the leg was fluid mixed with blood. When my leg was swollen, blood leaked even more. The nurse told that there had been some sort of mistake and I must spend one more night in the hospital on another unit, and in the morning someone will come after me. Next day around 10:00 am the driver came to pick me up to Hounslow Civic Center. Car was not adapted for disabled people. In Hounslow Civic Center car parking was far from the main entrance. I was not able to walk with a walking frame to the building. The driver arranged for me wheelchair. He took me into the building. Inside the building, the driver hended to a woman from reception of documents prepared by the social worker from the Royal London hospital The driver assured that I am in good hands and asked if he could leave me now. Receptionist said, he might leave me because I am safe here. When the driver was out of sight of the guards Hounslow Civic Center, they came to me, told me to give back wheelchair and leave the area because I have no right to be there I could not believe what was going on, it's a nightmare this cannot be happening. Tears flowed from my eyes, begged for help to call an ambulance because I was so week and unable to walk even with frame, I asked to call the police. They were merciless, forcing me to walk off the premises. I could barely leave the building, it was so dificult and painful for me. The main entrance is a lawn, I fell upon lawn and began to vomit from exhaustion. Security Guard came to where I was lying and they started to force me to go somewhere further into the park that people did not see me like this, vomiting and shaiking, wiyh fresh wound on leg. I went sat down on a bench on the flowerbed. It was very hot, did I faint every now and then vomited, and I was shaking like in a deep shock. Then I lay down in the shade on the dirty ground, because I was in very strong pain and I couldnt seat on the bench, I had so strong pulsating pain in my sick leg. I saw a fly walking around the wound on my leg, because from the stiches flow liqud mixed with blood. I was to week to protect my leg from insects, I did not care I wanted to die because incredible strong pain. I could not belive that institution who are to protect people refused to help me. It was like night mare. When I was lying on the dirty ground, I was almost near death, bodyguards of Hounslow Civic Centre repeatedly approached me and took me to all the documents. Then returned my documents. They said I could come in a few days to speak on my behalf. They also said that the document prepared by the Social Worker of the Royal London Hospital is invalid and I can put it in to the trash.

Leg was swollen and very hurt. I asked passers-by for help, to call the police and ambulance. I was in very strong shock, I vomit and had chills. I have not been able to think in English, so I cried in Polish. One Polish man heard my supplication. He heard my pleas for help, he asked what had happened? I told him my story, he could not believe it. I said, no one can believe it because this is too horrible to be true. But it is true!!! People think that such things may be only in the movies. The man offered me a car that will take me to the Polish Church on Ealing. He took me to this church but the priest's assistance was limited to only calling the police and an ambulance. My leg was red and swollen from the wound started drip oil. The priest said, that my leg looks awful and does not understand why everyone refuse to help. No one wanted to come to me I spent two days lying on the sidewalk outside the church, the priest and the good people brought me food and blankets. I was resigned to death. After two days came ambulance from Ealing Hospital. They examined my blood pressure and blood sugar levels. They was asked am I diabetic? I replied that no, I had never had a problem with blood sugar and no one in my family. I did test a few weeks ago in the Crisis Center of occasions health days and everything was normal, in addition when I was in Royal London Hospital I had all blood tests, they cant discharge me with hi blood sugar level and bad health condittion. After several days of hospitalization to stabilize blood sugar level, I was discharged from the Ealing Hospital. In Ealing Hospital they found that I have type 2 diabetes Mellitus and I have wound infection. Then because an infection of wound around a few times I was in University College Hospital. Then, for several months I have been treated in the Fracture Clinic, Mr. MJ Oddy Clinic, Lower Ground Floor, University College Hospital, 235 Uston Road, London, NW1 2BU. Through the intercession of Dr Nigel Hewett of Clinical Lead UCLH Homelessness Team, on december 2011 I got an accommodation, but far too late, because now I have a lot of complication because of strong stress, trauma and dearty environment because I had been not placed in temporary accommodation on right time, when I was discharge from the Royal London Hospital, I repeatedly dodged repeated infections. I probably would not have been caused by diabetes, depression, anxiety because chronic exposure to strong and long stress.

Please provide personal information (name, place of work) of Social Worker who did not ensure I was taken to temporary accommodation.

Please give the name of the driver who transported me to the Hounslow Civic Centre. Please let me know what kind of tests and blood tests were done to me when I was in Royal London Hospital. I mean the tests on blood glucose levels.
