When I first opened InDesign, I selected the Document option, and then changed the page size to from the default setting to A4.

I could give my document more or less columns from the same menu.

To add image boxes, I clicked on the tool on the right hand side and click and dragged the box where I wanted the image to go. It could snap to meet the edges of my gutters if I wanted it to.

To add the masthead I wanted to add it as a picture. To do this, I made sure it was saved as a jpeg file and then clicked on the image box went to File in InDesign and selected the picture from the pop up menu.
