For both citations of books and pamphlets and the bibliography, instead of italicized for title, it should be caps, small caps. I think there's a way of doing this in MS Word. Otherwise, sample is ok.

2. Format for periodical, journal articles is also ok except that when there is a series number aside from the volume number, my suggestion is to put the series number right after the volume number in parenthesis, e.g. 4(1) 3. The rest, esp. for primary materials (laws & cases) are ok as standard citation formats (same as in Feliciano's Philippine Manual of Legal Citations.)

4. A variation for electronic sources is to put the URL in brackets. However, we can also adopt Prof Mila's format."
