Requirements in the Issuance and Endorsement of STCW ’95 Certificate to Marine Deck Officers

I. Certificate of Registration/ STCW Certificate of Competency

II. Medical Certificate from DOH accredited medical clinic/hospital with QSS In place, using the DOH approved
seafarers pre-employment medical from, which must not be more than one (1) year from the date of
issuance. If submitted during application for STCW ’95 Certificates, should be not less than fifteen (15) days
prior to expiry date.
III. Transcript of Record with SO No. (For Category A only)
IV. One (1) year accumulated approved seagoing service during the preceding five (5) years.
A. Company Certification of Sea Service- shall include the type vessel’s , rank occupied, vessels gross tonnage,
vessel’s trading area and Inclusive dates of sea service ( letterhead with dry seal)
B. Seafarer’s I identification and Record Book (SIRB)- one photocopy each of pages one and two and other pages
reflecting sea service.( in case of loss/damage SIRB, any acceptable substitute documentary proof of sea service
and affidavit of loss shall be required.
V. National Assessment Center (NAC) Certificate for Regulations, V&VI

NOTE: NAC certificates under Regulation V Of the 1278 STCW w/ less than year validity shall be revalidated first from the
Issuing agency.

CATEGORY A- Marine Deck Officers who have commenced approved education training AFTER August 1, 1998.

1. Advance Fire Fighting-NAC

2. Proficiency in Survival craft and rescue Boat-NAC
3. Medical Emergency-First Aid-NAC
4. Personal Safety and Society Responsibility-NAC
5. Proficiency in Fast Rescue Boat (if applicant will board a ship equipped with fast rescue boat)
6. ECDIS ( if applicant will board a ship equipped with ECDIS)
7. GOC ( with at least one (1) year validity)
8. SSTB practical assessment

CATEGORY B- Marine Deck Officers who are holder of STCW ’78 Certificates in the operational Level AND have NOT been
previously Issued STCW’95 Certificates.

1. Basic Safety Training-NAC

1.1Personal Survival Techniques
1.2 Fire Prevention and Fire Fighting
1.3 Elementary First Aid
1.4 Personal Safety and Social Responsibility
2. GOC (with at least one (1) year validity)
3. RSC- Radar Navigation, Radar Plotting and Use of ARPA
5. Advance Fire Fighting-NAC
6. Proficiency in Survival Craft and Rescue Boat- NAC
7. Medical Emergency First Aid- NAC
8. Ship Simulator & Bridge Teamwork (Except those who have taken and passed the practical exam)
9. Proficiency in Fast Rescue Boat ( applicant will board a ship equipped with fast rescue boat)
10. ECDES ( If applicant will board a ship equipped with ECDIS)
11. Cargo Handling and Care of Cargo
12. MARPOL 73/78 Annexes 1&2
13. Collision Avoidance
14. SSBT practical assessment

CATEGORY C- Marine Deck Officers who will board seagoing ship requiring special training.

1. For Tanker Regulation V/I

a. Three (3) month approved seagoing service on tanker OR General Tanker Familiarization- NAC
b. Shore- based Fire Fighting
2. For RO-RO Passenger Ship ( Reg. V/3)
2.1 Crowd Management, Passenger Safety and Safety Training for Personnel Providing Direct Services to Passengers
in Passenger Spaces (IMO Model Course 1.28)- NAC
2.2 Proficiency in Crisis Management and Human Behavior Training in Passenger Safety, Cargo Safety Hull Integrity
Training (IMO Model Course 1.28)- NAC
2.3 Familiarization Training on Board RO-RO (Reg. V/3)
