Disclosure Statement

Union Middle School Reading 2013-2014 1. Course Philosophy Reading is one of the most important skills for success in school and in life. I have designed this class to help students improve their reading skills while developing decision-making techniques, group cooperation and responsible behavior. 2. Goals: Students will work on improving their fluency, decoding, vocabulary and comprehension skills. 3. Academic Grades: Grades in this class will be based mostly on participation in class activities, also on assignments, assessments and outside reading logs. Grading scale is as follows: 100-90 A, 89-80 B, 79-70 C, 69-62 D, 58-61 D-, 57-0 F 4. Citizenship grades: are based on attitude and respect as well as punctuality. Students who obey class rules almost all of the time will receive H's. Students who follow rules most of the time receive S's. Students who require frequent reminders to remain on task and courteous receive N's, and students who frequently disrupt the learning of others receive U's. The citizenship grade also drops a letter for every three tardies. 5. Make-Up Work for Absence A student will be given the opportunity to make up work missed. It is the responsibility of the student to request and complete any assignments or tests missed because of absence. Makeup work is due within five days of the absence. (For longer absences, arrangements may be made on an individual bases with the teacher). 6. Class Rules:
Respect your teacher and classmates, this includes: 1. Coming to class on time and prepared with paper, pencil and planner. 2. One person speaks at a time. If you have a question or comment, raise your hand and wait to be recognized. 3. Follow all instructions immediately. 4. Keep your hands, feet, mouth and objects to yourself. 5. Do not argue with the teacher. If you break a rule we will follow the Union Middle School classroom management policy: 1. The teacher will give a verbal warning 2. Student will be held after class 39 seconds 3. Student will stay after an additional 1 minute 16 seconds 4. Think Time and home contact 5. Referral to administration

In the event that a student has problems beyond his/her control, which are not covered by this statement, the teacher may skip steps to deal with the situation, keeping in mind the best learning environment for the entire class. She may also meet with the student, and if necessary, the students parent/guardian, in order to come to an equitable solution.

Zero tolerance ABSOLUTELY NO USE OF CELL PHONES, MUSIC DEVICES, OR OTHER ELECTRONIC DEVICES WITH OUT SPECIFIC PERMISSION FROM THE TEACHER. NO WARNINGS WILL BE GIVEN FOR THESE THEY WILL BE TAKEN AND TURNED IN TO THE OFFICE.

6. Getting Help Outside of Class I am happy to help students and meet with parents. As I am also the schools reading specialist the best way to contact me for help outside is to make an appointment. My office is room 102 and I will be teaching in room 402, Ms. Crandalls room. Students who need additional help may arrange for a time with me at least one day in advance. 5. Dealing with Unusual Circumstances In the event that a student has problems beyond his or her own control which are not covered under this disclosure, I will meet with the individual student, and if necessary the students parent or guardian, in order to come to a reasonable solution.
