Discharge Plan
Medication: The patient was prescribed and instructed by the physician to take medications at the right timing and dosage. Take home medications include omeprazole capsule, tramadol tablet, ciprofloxacin tablet and vitamin C tablet. Report or consult to the physician if adverse reactions occur. Encourage significant others to assist the patient during medication administration and to encourage her to increase fluid intake to prevent further harm to the patients kidney and liver. Health Teaching: Patient was instructed to increase fluid intake to prevent gastric discomfort due to the medications and to comply on continuous wound care. Reduce intake of saturated fat, increase fiber intake and decrease intake of sugar. Instruct the patient and the significant others to report any unusualties and occurences of reactions due to drug therapy to the physician in order to prevent complications and further development of health problems. Significant others was informed to be compliant on the health teachings by assisting the patient maintain a healthy lifestyle.
