Pay LSG fee (PhP100) and Batch Fund (PhP100) and sign on wherever necessary c. Wait for name to be called, and get back your Form5A 2. OCS a. When name is called, go to the OCS b. Get your TCG c. Give your TCG with your Form 5A to the RegCom Member stationed there d. Inform the RegCom Member whether there are changes to be made on your classes (ALL YOUR CLASSES ARE ALREADY IN YOUR FORM 5A) or not e. Go to Sarmiento Room and wait for name to be called 3. Sarmiento Room a. If there are no changes to be made i. when your name is called, proceed to the OCS to get assessed and validated ii. after your Form 5 is printed, sign it where necessary iii. Give your SIGNED Form 5 to the RegCom Member stationed at the Colleges Secretarys Office iv. Go back to Sarmiento Room and wait for your name to be called v. When your name is called, you may now proceed to Payment b. If there are changes to be made i. If you got all your subjects in the pre-appeals 1. Wait in Sarmiento Room while your classes are encoded 2. when your name is called, proceed to the OCS to get assessed and validated 3. after your Form 5 is printed, sign it where necessary

4. Give your SIGNED Form 5 to the RegCom Member stationed at the Colleges Secretarys Office 5. Go back to Sarmiento Room and wait for your name to be called 6. When your name is called, you may now proceed to Payment ii. If you are still lacking subjects 1. When your name is called, proceed to Soliven Room for Subject Appeals 2. Go back to Sarmiento Room and wait for your name to be called 3. When your name is called, proceed to the OCS to get assessed and validated 4. after your Form 5 is printed, sign it where necessary 5. Give your SIGNED Form 5 to the RegCom Member stationed at the Colleges Secretarys Office 6. Go back to Sarmiento Room and wait for your name to be called 7. When your name is called, you may now proceed to Payment *If you have 4.0 or a 5.0 or dropped a classes last semester, expect to take longer as there is a big chance the College Secretary will want to talk to you. *Subject Appeals are on a first-come first served basis(?) *For Enrolment by Proxy, be sure to give your Proxy Letter and ID to your Proxy as the OCS may not release your TCG to your proxy without these *Remember to Print your Form 5A.
