Task Analysis Carter Smith Task Analysis Student Information Carter Smith 7 years old 2nd grade Diagnosed with a Traumatic Brain Injury (TBI) Carter has Tuberous Sclerosis Complex (TSC), a genetic disease in which nonmalignant tumors grow in the persons brain, and other vital organs. Carter has had several of these tuberous tumors removed from his brain. The tumors caused Carter to

have seizures. The seizures have caused Carter to have several motor issues. He has partial paralysis on the right side of his body, and suffers from tremors in his arms. The tumors, and the surgery to remove those tumors, have caused Carter to be cognitively impaired. Carter has difficulties with the morning arrival procedures. He is unable to properly sequence taking off his coat and backpack, finding his back and forth book and lunchbox in his backpack, and putting his lunch box and back and forth book in the appropriate places.

Goal When arriving in the classroom, at the start of the school day, Carter will complete the correct sequence of the classroom morning arrival procedures, with no prompting, 5 out of 5 days for 4 weeks, as measured by a teacher-made observation checklist. Objective #1: When arriving in the classroom, at the start of the school day, Carter will hang up his backpack and coat, with no prompting, 5 out of 5 days for 4 weeks, as measured by a teacher-made observation checklist.

Task Analysis Objective #2: When arriving in the classroom, at the start of the school day, Carter will hang up his backpack and coat and retrieve his lunchbox and back forth book out of his backpack with no prompting, 5 out of 5 days for 4 weeks, as measured by a teachermade observation checklist. Objective #3: When arriving in the classroom, at the start of the school day, Carter will hang up his backpack and coat, retrieve his lunchbox and back forth book out of his backpack, and place his lunch and back and forth book in the correct place within the classroom with no prompting, 5 out of 5 days for 4 weeks, as measured by a teachermade observation checklist.

Task Analysis of Objective #1 Date

1. Walk to the cubby labeled CS. 2. Take the backpack off of my back. 3. Hold my backpack by the handle at the top. 4. Place the handle of the backpack on the hook. 5. Take my coat off of my body. 6. Hold my coat by the hood. 7. Place the hood of my coat on the hook. Comments: The adult will provide prompts from least-to-most intrusive. The adult will mark which prompts were used, and the success of the task completed, in the column under the appropriate date.
