Case Based IEP Assignment *We determined that Jorge would need more than minimal support when in the general education classroom 75% of the time.* 1. LRE Statement: Due to his present level of performance, Jorge needs one-onone instruction in reading, writing, math, and social skills. He will receive support from the special education teacher and para-professionals in both the general education classroom and resource room (ILC). 2. Goals: a. Math Goal: When given two groups of math manipulatives, Jorge will count the number of manipulatives in each group and then combine the groups to find the sum of the two groups. Jorge will correctly count and combine the groups, 4 out of 5 times for 4 weeks, as measured by a teacher-made checklist. i. Math Objective #1: When given one group of math manipulatives, Jorge will correctly count the number of objects in the group, 4 out of 5 times for 4 weeks, as measured by a teacher-made checklist. ii. Math Objective #2: When given two groups of math manipulatives, Jorge will correctly count the number of objects in each group, 4 out of 5 times for 4 weeks, as measured by a teacher-made checklist. b. Writing Goal: When given 3 sentence stems, Jorge will correctly complete each sentence stem, and then take each sentence and place it in the appropriate area of a graphic organizer. Jorge will correctly do this 2 out 3 times for 6 weeks as measured by a teacher-made checklist.

i. Writing Objective #1: When given 3 sentence stems, Jorge will correctly complete each sentence stem, 2 out 3 times for 6 weeks as measured by a teacher-made checklist. ii. Writing Objective #2: When given a graphic organizer, Jorge will correctly complete all areas of the graphic organizer, 2 out 3 times for 6 weeks as measured by a teacher-made checklist. c. Communication Goal: When given the use of a timer during a preferred activity, Jorge will comply with the staff members request within 30 seconds after the sound of the timer, without shouting or using swear words. Jorge will complete this 4 out of 5 times for 4 weeks as measured by a teacher-made checklist. i. Communication Objective #1: When given the use of a timer during a preferred activity, Jorge will comply with the staff members request within 1 minute after the sound of the timer, without shouting or using swear words. Jorge will complete this 4 out of 5 times for 4 weeks as measured by a teacher-made checklist. ii. Communication Objective #2: When given the use of a timer during a preferred activity, Jorge will comply with the staff members request within 1 minute after the sound of the timer, without shouting or using swear words.
