Presenting Problem Chris Ross (pseudonym) is a 21 year old single Caucasian male. He has never been married and currently lives with his parents. Chris has been employed as a machine worker at Paslin Co. for the past two years. Chris is coming to see a social worker because of stress resulting from a previous relationship which has emotionally been taking a toll on him as well. Family of Origin History The client reports that his mother and father were 35 and 32 years old, at the time of his birth. His parents were married for 13 years at the time and did not plan on having another child at the time. During his childhood, Chriss father was a tool and dye worker at Paslin while his mother stayed at home taking care of him and his brother. The clients father is now 56 years old. Chris reports that his mother took some college courses while he was growing up but never pursued any type of degree. She is currently 53 years old and works as a manager at the bounce house. The client has 1 sibling from the same marriage. His brother Jason is currently 30 and is physically disabled and cannot work due to an accident while in the Marine Corps. Jason is married with five children, two of which are not his biologically. Chris reports that he loves his brother and they were very close while growing up, but with a 10 year age difference, their relationship is now strained because of their busy lives. The client reports that both his parents never completed college, but they did take some various courses throughout their lives at the local community college. His brother never obtained a degree either because he decided to go into the Marine Corps at the age of 18. Chriss brother is in good health, but has trouble walking around due to his previous injury. Chris reports that while growing up he was a very happy child and his household was happy as well. He was very close to his mother because she was a stay at home mom and was always

there to play with and talk to. His father was always at work, so his mother was his primary emotional and physical support. The client has a fairly small family. On holidays and other family events, they get together at his house with his brother, his sister-in-law, and their children. The family celebrates major holidays such as Christmas, Thanksgiving, and Easter this way. They would make a lot of food and play various games at these events to keep the kids occupied. On Thanksgiving, they celebrate by having family over and talking about what they are grateful for in their lives. Independence day is also celebrated by going to downtown Mt. Clemens to see the fireworks and having a small get together at their house after. Deaths are usually acknowledged by small funerals and they are very open with their emotions. When grieving they talk about good times of the deceased and never mention any bad times. Chris does not report any physical or sexual abuse within his family. Developmental History Chris was delivered vaginally when his mother hit the full-term mark of her pregnancy. He was 6 pounds 11 ounces at birth and there were no complications as far as he is aware. The client was breastfed for one week and then bottle fed after until he was about one year old. From Chriss knowledge he had no developmental delays and he was more advanced than his brother in walking. He began walking at 10 months while his brother started walking at 11 months. Health History The client reports that he had good health at birth and throughout infancy. As a child, he caught pneumonia for four days and he recalls them being his worst days as a child. Chris also jumped out of a tree at the age of 5 and broke his pinky toe and when he was in the sixth grade he broke

his pinky finger playing volleyball. Other than that, Chris does not report having any other childhood accidents. Chris reports that he has always been average in size and does not have any issues when it comes to weight or food. The client does not report having any illnesses or surgeries since his childhood. Chris reports that there are no chronic physical diseases on either his mother or fathers side. Substance Use/Abuse History The client reports that he currently smokes cigarettes, but only smokes occasionally, and has done so since he was 16. Chris reports that he smokes marijuana everyday and started when he was 17. He also reports that he was been drinking alcohol since he was 18, but he now drinks more because he just turned 21 a couple of months ago. He drinks about three to five beers after work about three to four times a week. Chris reports that he has also experimented with other drugs within the past couple of months, specifically Aderal and Morphine. He claims to take Aderal to help him stay up throughout his work day and he has only taken Morphine once which was a 50mg pill. The client states that he has never sought out treatment for any type of substance abuse. Mental Health The client states that as a child in the second grade he saw a psychiatrist one time. He states that his second grade teacher wanted his parents to put him on Ridalin but his mom never pursued it after the psychiatrist evaluation. Other than that one time, Chris reports to have no other Mental Health treatment. Relationships/Sexual History

Chris reports that he had a lot of friends as a child. He mainly hung out with his next door neighbor Jake, but they no longer talk. Due to his previous girlfriend, he stopped hanging out with most of the friends that he has but he still has one good friend that he has kept in contact with. He states that because of losing his friends, he feels alone and fears that he will always be alone. The client is heterosexual and began dating in middle school. In his teenage years, he began hanging out with some bad influences and began dating some of the girls within the group. Chris states that he started having sex at the age of 17 and was in a committed relationship with that girl for three and a half years. They then broke up and he began randomly having sex with random girls to try to get her off of his mind. He states that he can have orgasms and is fully functioning at that level. The client reports that he has never used any method of birth control with any of his partners because he believes that it is not needed. He wants children in the future but not currently at this time. He currently has no sexually transmitted diseases and never has as well. Chris reports to continue to randomly have sexual relationships while looking for a committed relationship, but all attempts have failed so far. He explains that he has never felt the same way about another girl since his breakup with his first love. Family of Creation History The client has never been married, but has been in one committed relationship. He states that he has no children but plans on having children in the near future when he finds his soul mate. Chris states that his committed relationship went bad because his girlfriend had cheated on him with his best friend. He is now emotionally torn because he cannot move on from it but he still

loves her and wants to be with her. They are currently broken up but are still communicating and trying to make it work. Military History The client denies having any military experience. Financial History Chris states that while growing up his parents were financially stable. He had everything he needed and they lived comfortably. The client states that now his finances are bad because he spends most of his money on marijuana Legal History The client denies any legal history. Immigration Issues Chris denies any immigration issues. Religious History The client reports that growing up he was raised Baptist. He reports that he would only go to church once a year with his mother and father which was always on one of the major holidays. Chris reports that at this time he does not attend church and it is not because of any problems with the Baptist religion. Educational History Chris claims that he was always good in school. He never struggles in elementary, junior, or high school and was a pretty average student. The client had no behavior problems and never was marked down for any behaviors. He graduated high school and started going to Macomb Community College at the age of 18. Chris states that he took basic core classes for three semesters but then decided to stop going due to work. The clients states his desire to go back to

college to become a nurse because he believes that he is generally smart and is confident to do well in the program. Employment History The client reports that growing up he had basic chores such as cutting the grass, taking out garbage, and plowing snow. Currently Chris pays rent to his parents and does not have to do any chores while living there. In December of 2009, Chris reports that he started working at Burger King and ended up leaving in July of 2011 for another job opportunity. He reports that now he works for Paslin Co. and has been since July of 2011. Chris states that he generally does not enjoy working and he changed jobs because Paslin offered him triple the amount of money that he was making at Burger King. The client reports that he now makes $40K a year and has never been trained at Paslin for the job that is required of him. Strengths Chris reports that his only strength is being very athletic. By interviewing him, I have found him to be very closed off and not very proud of his strengths. I have noticed that one strength of his is that he can talk to anyone. Chris can start a conversation and talk about a topic with someone for a long time. Family wise, the client states that he has a very good support system. If he needs something, he says that he just asks and they are there for him. He is more close to his father now because they work together, but his mother is always emotionally and physically there for him. His relationship with his brother is strained because he has his own life going on, but when they get together, Chris reports that they get along and they can talk about anything.

As far as friends go, Chris did state that he has one very close friend. He reports that this one friend is there for him emotionally and physically and he knows that he can talk to her about anything. The client seems to keep his friends and family dear to his heart. He seems to be happy and healthy and he is very open to the idea of me helping him with his emotional problem.

Step 1: Working with the client Mr. Warnez discussed with me some of the issues that he is experiencing at this time and he would like to address and work on. The client describes the issues as the following: Issues: 1. The client is experiencing a lot of conflict with his ex-girlfriend regarding their relationship. 2. He is experiencing persistent stress related to a previous relationship break-up. 3. He wants to get this addiction to marijuana under control but does not know how to approach it. 4. He is falling into a lonely of mind because he has no friends. Step 2: Prioritize the Problems 1. Conflict with his ex-girlfriend 2. Presistent stress 3. Uncontrolled addiction to marijuana 4. Lonely state of mind Step 3: Translate the Problem into Need Problem 1. He is having conflict with his ex-girlfriend 2. He is having persistent stress 3. Addiction to marijuana 4. Step 4: Evaluate Levels of Intervention for Each Need Need #1: Decrease conflict with his ex-girlfriend Micro Strategy: Recommend a book about couple relationships from a book store. Pros: He can learn new ideas to help manage the conflict with his ex-girlfriend

The book may be a fast way to obtain help regarding their conflicts

Purchase the book if it is available within 7 days. 4. The client will read the book and use one strategy to cope with the arguing for the next week. B. Obtain couples counseling within 1 week 1. The client will not hang out around people that smoke marijuana within 5 days. 3. The client will not let anyone influence him to smoking marijuana within 7 days. B. The client will attend substance abuse classes. 1. He will search for substance abuse classes in his area within 3 days. 2. He will attend the required meetings within 7 days. 3. He will use strategies learned to quit smoking within 3 weeks. Goal # 4: The client will feel less lonely. A. He will make an effort to get out and meet new people. 1. The client will search different clubs and gyms to join within 3 days.
