    AFFIDAVIT OF DISCREPANCY
    I, JOCELYN ASUNCION, of legal age, Filipino and with residence address at 2601 Centennial II-B Noah Road Nagpayong Pinagbuhatan ,Pasig City, after being duly sworn to according to law, hereby depose and state: 1. That I was born on October 27, 1981 at San Pascual, Masbate; That there is a discrepancy in my Marriage Contract and Birth Certificate specifically in the entry of my parents name; That, appearing in the entry of my Birth Certificate, the name of my father is JULITO LUCHAVEZ and appearing in the entry of my Marriage Contract, the name of my father is DEOLITO LUCHAVEZ; That the name JULITO LUCHAVEZ as appearing in my Marriage Contract and the name DEOLITO LUCHAVEZ as appearing in my Birth Certificate refers to one person; That, the name of my mother appearing in my Marriage Contract is ADELIA LUCHAVEZ and the name of my mother appearing in my Birth Certificate is DELIA LUCHAVEZ; That the name ADELIA LUCHAVEZ as appearing on my Marriage Contract and the name DELIA LUCHAVEZ as appearing on my Birth Certificate refers to one person;

    That in my Marriage Contract the name of my mother is ADELIA QUILOAN and in another entry is ADELIA Q. LUCHAVEZ; That ADELIA LUCHAVEZ and ADELIA QUILOAN as appearing in the different entry of my Marriage Contract refers to one person; That such discrepancy was due to mechanical error of writing; That I am executing this Affidavit in support of my application for a new Philippine passport;
