Instructions:
To get this limited-edition library card while supplies last, please complete this form and submit it with an acceptable ID to your local public library. Adult students can show a current New York State drivers license, learners permit, or other photo identification. Young Adults (ages 1316) can show current working papers or school ID. Children (ages 12 and under) must have a parent or legal guardian sign this form; parents will be responsible for materials checked out on their childs card. Visit the Web site of the library in your borough for a complete list of acceptable identification forms and the location of a branch near you. For Brooklyn, go to brooklynpubliclibrary.org; for the Bronx, Manhattan, and Staten Island, go to nypl.org; for the Queens go to queenslibrary.org.

The New Library Card: What you need to know.


There are more than 200 public libraries throughout New York City. A free library card will allow you to borrow books, ebooks, DVDs, and CDs, and access exclusive online resources for cardholders. Visit your local library with this form completed to get this new card while supplies last. Students 12 and younger need a parent or guardians signature on the form. Even if you have fines on your existing library card they wont carry over to the new card. Get your new card (while supplies last) at any Brooklyn Public or New York Public library by August 31, 2012 to get this benefit at BPL or NYPL. Its a fresh start! ID is required. Check with your local library to find out what to bring with you when you visit. The new card is only available to children and students birth to grade 12. Have questions?
