Building on the excellent results obtained with the St. Georg Hinge Knee Prosthesis, in 1979 the Rotational Knee Prosthesis was developed, which allows axial rotation and reduces the forces acting on the prosthesis anchorage. The intracondylar Endo-Model Rotational Knee Prosthesis is available in two versions and four implant sizes (right and left): A Model without patella flange for primary implantation preserving the bony femoropatellar joint Model with patella flange for use when replacing the femoropatellar joint and for revisions

Retaining the low friction principle, the physiological movement of the Rotational Knee Prosthesis is optimal because the pivot point is within the physiological area. Flexion and rotation of the Rotational Knee Prosthesis take place in a cross joint.

System Description
A cada paso, y especialmente en caso de cada, se originan tensiones de momentos de torsin que actan sobre el anclaje del implante, con un efecto negativo sobre la duracin de la prtesis. La transmisin elstica de fuerzas hecha posible por la estructura de la prtesis protege las interfases cemento seo prtesis y cemento seo hueso. Debido a las favorables dimensiones de la Prtesis de Rodilla Rotacional, se requiere una muy pequea reseccin en el plano de la articulacin tibiofemoral, solamente 14 mm (Fig. 1). El tamao de la porcin intracondilar depende del tamao del implante, pero es solamente de 28 a 34 mm (Fig. 2). Esto constituye un importante punto positivo en trminos de la posterior ciruga de revisin. Over-extension amounts to 3. The Endo-Model Rotational Knee Prosthesis allows flexion of up to 165. In addition, the kinematics of this design provide physiological rotation, with elastic transmission of forces enabled by the special shape of the tibial running surface. At every step, and even more in the case of a fall, torsional stresses arise and act on the implant anchorage, with a negative effect on the lifespan of the prosthesis. The elastic transmission of forces allowed by the construction of the prosthesis protects the bone cement /prosthesis and bone cement / bone interfaces. Because of the favorable dimensions of the Rotational Knee Prosthesis, the resection required on the tibiofemoral joint plane is very small only 14 mm (Fig.1). The size of the intracondylar portion depends on the implant size but is only between 28 and 34 mm (Fig. 2). This is an important positive point in terms of subsequent revision surgery.

The dimensions and shape of the Rotational Knee Prosthesis allows a good overview of the operative field. The femoral and tibial components are coupled by simply slotting the two parts together (Fig. 3). The prostheses feature an anti-dislocation device (Fig. 4). Implantation is made easier by just a few easy-use instruments.

In knee replacement, advancement of the patella or of the patellar bearing surface is often observed. By moving the femoral component dorsally relative to the tibial axis, physiological movement is achieved in the femoropatellar joint as well. This protects against progression of retropatellar arthrosis. Rotation of the prosthesis ends in extension by form closure, which ensures a secure standing position. Rotation increases continuously with flexion. This rotation is limited primarily by the capsule-ligament apparatus (Fig. 5).

Both prosthesis components are broadly supported on their corresponding joint surfaces, such that the compressive strength of the cancellous bone is not exceeded. System Description
Los vstagos protsicos incrementan la seguridad de la alineacin. La seccin transversal es rectangular, con grandes radios de transicin y sin bordes afilados. Centradores de polietileno en forma de estrella al extremo de los vstagos aseguran que cada vstago est posicionado centralmente en el canal medular (Fig. 8), evitando as todo contacto directo entre el vstago metlico y la cortical interna.
Cuando los componentes son extrados del lecho de cemento, la estrella de centrado generalmente se rompe y puede extraerse mediante brocado en una segunda fase. Si el mecanismo articular de la prtesis est desgastado, por ejemplo, en el caso de alineacin indebida de la prtesis, sta puede cambiarse en una operacin de revisin sin necesidad de extraer el componente femoral o tibial.
The Endo-Model Rotational Knee Prosthesis offers optimum security of implant anchorage. Because the stem has no surface structuring at all, there is nothing to hinder extraction of the prosthesis during a later revision procedure (Fig. 9). When the components are being knocked out of the cement bed, the centralizer usually breaks off and can then be drilled out in a second step. If the cross joint is worn out, e.g., in the case of a malaligned prosthesis, it can be exchanged in a revision operation without the need to remove the femoral or tibial component.

The prosthesis stems increase the security of the alignment. The cross-section is rectangular with large transition radii and no sharp edges. Star-shaped polyethylene centralizers at the end of the stems ensure that each stem is centrally positioned in the medullary canal (Fig. 8), thus avoiding any direct contact between the metal stem and the inner cortex.

The external shape, dimensions and sizes of the Endo-Model Hinge Knee Prosthesis correspond to those of the Endo-Model Rotational Knee Prosthesis. As the implant beds required for the hinged and rotational versions are identical, the decision whether to use a rotational or a more stabilizing hinged knee prosthesis can be made intraoperatively. Connecting piece A, which is fixed to the tibial component and links it to the femoral component of the Hinge Knee Prosthesis, features a borehole for the joint axis B. The ventral borehole C is provided for the set screw D, whose tip fits into the borehole E on the axis. Once the upper and lower components have been joined, the axis is locked with the headless screw.

System Description
From inside the intracondylar box, polyethylene bearings for the prosthesis axis are pushed into the medial and lateral boreholes. The upper and lower prosthesis components are joined by introducing the tibial connecting piece into the intracondylar box of the femoral component, such that the prosthesis axis can be inserted (always from the medial aspect) using the Threaded Rod. Articulation takes place between the prosthesis axis and the two bearings. The Endo-Model Hinge Knee Prosthesis is delivered ready assembled and in a sterile condition. To disassemble it, turn the set screw D counterclockwise. Screw the Threaded Rod onto the prosthesis axis B, which is then pulled out. Push the bearings F of the upper prosthesis component into the intracondylar box and remove them. (Note that the open bearing must be placed medially when the bearings are reinserted). The package contains two sterile Trial Bearings (not autoclavable). These are inserted into the upper prosthesis component during surgery; after the trial run, they are exchanged for the definitive bearings. These too can be exchanged if necessary in a second intervention.

Las indicaciones de los diversos tipos de sistemas de prtesis de articulacin de rodilla dependen del estado de los elementos estabilizadores pasivos y activos (ligamentos y cpsula, msculos y tendones), de la extensin de la deformacin de la articulacin existente, de la patologa de base, del tipo de paciente y de la experiencia del cirujano. Si la aplicacin de una prtesis de rodilla unicondilar o de sustitucin de superficie total no est justificada debido a las deformaciones existentes y a la inestabilidad de la articulacin, pueden resultar adecuadas las ventajas de la estabilizadora Prtesis de Articulacin de Rodilla Total Endo-Model. Indications for the various types of knee joint prosthesis systems depend on the state of passive and active stabilizers (ligaments and capsule, muscles and tendons), the extent of the prevailing joint deformity, the basic illness, the type of patient and the experience of the surgeon. If the application of an unicondylar or total resurfacing knee prosthesis is not justified due to the present deformities and joint instability the advantages of the stabilizing Endo-Model Total Knee Prostheses can be used. Sustitucin Femoral Total Endo-Model Total Femur Replacement Endo-Model
La Sustitucin Femoral Total se prepara en funcin de la sustancia sea existente y las medidas especficas del paciente. A tal efecto hay disponibles formularios especiales para que el cirujano introduzca las medidas y el nombre del paciente. La articulacin de la rodilla va provista de una Prtesis de Rodilla Rotacional o de Charnela, con o sin superficie rotuliana. La unin slida a la articulacin de la cadera se produce mediante un vstago insertado a travs. El componente del cuello con cono se monta en el vstago insertado y permite el uso de cabezas femorales o, en el caso de prdida del acetbulo, de una prtesis en silla de montar. Ms informacin disponible si se solicita. The modular Total Femur Replacement is manufactured on basis of the remaining bone stock and patient-related measurements. For this purpose special forms are available for the surgeon to enter the measurements and the patients name. The knee section features a Rotational or Hinge Knee Prosthesis with or without patellar flange. The solid connection to the hip joint is provided by a push-through stem. The neck component with taper is mounted on the push-through stem and allows the use of femoral heads or in case of loss of the acetabulum of a Saddle. Further information available on request.

T-axis (cross) without removal of implants In case of a damaged joint mechanism of an implanted prosthesis or in case of joint instability provided that the implant components are well fixed, the Total Knee System offers the following solutions: exchange of the bushing of a Rotational Knee Prosthesis complete exchange of the T-axis (cross joint), if a Rotational Knee with an anti-luxation device or a Hinge Knee is indicated. In the latter case, the tibial components must be replaced, however. exchange of the axis or the polyethylene bearings in a Hinge Knee Prosthesis. Al utilizar nuestros implantes se debe tener en cuenta lo siguiente: 1. Es extremadamente importante elegir el implante correcto. El tamao y la forma de los huesos humanos condicionan los del implante. Esto condiciona tambin la capacidad del implante de soportar cargas. Los implantes no estn diseados para resistir unas cargas corporales ilimitadas. La exigencia fsica sobre el implante no debe superar las cargas funcionales normales. 2. Es tambin muy importante manipular adecuadamente los implantes. Alterar la forma de un implante acabado reduce su duracin. Debe evitarse siempre lo siguiente: doblar en ngulo agudo el implante, doblarlo despus en sentido contrario o hacer ranuras o provocar araazos en l. Nuestros implantes no pueden combinarse con otros de distintos fabricantes. 3. Los implantes no deben reutilizarse. Aunque un implante utilizado no presente ningn deterioro externo, debe considerarse que el material ha sufrido una fatiga interna. 4. Tambin es muy importante la rehabilitacin del paciente, que debe estar informado de las limitaciones del implante. Un implante no es equiparable a un hueso sano en cuanto a su capacidad para soportar cargas. 5. A menos que se indique lo contrario, los implantes se suministran en envase estril. En el almacenamiento de implantes envasados es importante tener en cuenta lo siguiente: Evitar cambios extremos o rpidos de temperatura Se recomienda una temperatura de almacenamiento de 18-20C con una humedad del 50-70% Evitar la luz directa del sol Proteger los implantes de la humedad y del deterioro mecnico Los implantes pueden ser almacenados hasta 5 aos en su envase original. La fecha de caducidad va indicada en la etiqueta del producto No se debe utilizar un implante si su envase aparece deteriorado 6. 2. It is also very important to treat implants correctly. Altering the shape of a finished implant reduces its lifespan. The following must be actively avoided: bending the implant sharply, bending it back, or making grooves or scratches in it. Our implants may not be combined with implants from other manufacturers.
3. Implants must not be re-used. Even if a used implant looks undamaged externally, it must be assumed that the material has become fatigued internally. 4. Patient rehabilitation is also very important. The patient must learn about the limitations of the implant. An implant cannot compare with healthy bone in the ability to withstand stress!
5. Unless otherwise stated, implants are shipped in sterile packaging. The following are important for storage of packaged implants:
Avoid extreme or sudden changes in temper-ature. We recommend a storage temperature of 18 20 C with 50 70% humidity. Avoid direct sunlight. Protect implants from damp and from mechanical damage. Implants may be stored for up to 5 years in their original packaging. The Use by date is on the product label. Do not use an implant if its packaging has been damaged.
6. The documentation trail is important. Please use the documentation sticker included in the packaging for this purpose.
