Lesson Plan and Instructional Program Format Moderate Lesson Plan Student: Ethan Date:

Program: _________________ Objective: Ethan will place a red, yellow, or green sticker on a graph to show his level 6 times throughout the day. If Ethan is on the yellow or red level, Ethan will take a calm down break. Instructional Procedures: Setting, Grouping, Mrs. Dotsons room, Ethans desk, possibly the ALE Positioning

Equipment/Materials Instruction and Prompt

Premade Graph for zones, Ethans desk, ALE I will review the different levels with Ethan, how he feels during them, what he might be doing, and what others may be doing around him during the stages. I will explain the graph to Ethan. Ethan will have 6 different times throughout his schedule where he will have to evaluate his behaviors and what level he is on. Depending on his level, Ethan will be asked to make the appropriate decision. Appropriate decisions: Green- continue doing his work and following his schedule Yellow- Take a calm down break in the ALE Red- Take a calm down break in the ALE Prompts: Ethans schedule will serve as his first prompt. When Ethan sees Graph zone on his schedule, he will be expected to walk over to the graph, think about how he feels, and put it on the graph. Verbal Prompt- If Ethan does not do his graphing, he will be given one verbal warning. Physical Prompt, If Ethan does not complete the task after a verbal prompt, he will be given the option to walk to the ALE or

to be assisted to the ALE.

Correct Response Time Delay and Correction

Ethan will graph his zone level and, if needed, take himself to the ALE to complete a calm down break with the door open. Ethan will be given 5 seconds to start the task. If Ethan does not start the task, he will be given the least restrictive prompt. (calendar, verbal, physical)

Ethan will check his zone levels 6 times a day. A teacher or para will be checking to make sure Ethan is accurately assessing his own zone. Data will be taken every time Ethan graphs his zone. The data will ask whether or not Ethan was able to complete the task and whether or not Ethan was able to complete it accurately. If Ethan is able to identify the correct zone for his behaviors, than the number of times he graphs will go down to 3. If Ethan is not able to identify the correct zone for his behaviors, Ethan will have a review of what the zones are and have to graph his behavior 2 more times per day, so he can accurately identify his behaviors.
