Phone conversation with practicum instructor, Sandy Doctoroff, Ph.D., to discuss individualized practicum activities and competencies to be addressed; indirect work. Rearranged my online portfolio to be able to easily navigate between multiple classes. Updated personal information on the portfolio and added SEI 513; indirect work. Began constructing my Individualized Practicum Plan (IPP); indirect work. Finished my IPP and submitted; updated electronic portfolio; indirect work. Prepared for a six month review by gathering and organizing documents such as the Individualized Family Service Plan (IFSP) including the signature page and information pertaining to the Part C to Part B transition; indirect work. Facilitated a six month review of services; discussed outcomes, created new outcomes, discussed the transition from Part C to Part B process; direct work. Updated the IFSP from yesterdays six month review, as well as correcting things from a previous review before I took this position; gathered documents and passed information on to the Part B transition coordinator; indirect work. Prepared for a Battelle Developmental Inventory-II (BDI-II) to be administered tomorrow; read through items in the two sections I will be administering communication and cognition; photocopied the parts of the booklet I need to complete my section; indirect work. Wrote the Written Notice pertaining to yesterdays evaluation; scored my sections of the evaluation; conferenced with the Occupational Therapist I administered the BDI-II with about eligibility; called the family to discuss the results of the evaluation and to schedule an eligibility meeting; direct and indirect work. Facilitated an eligibility meeting; wrote the Written Notice for the meeting; direct and indirect work. Facilitated a six month review for a family who recently moved into my catchment area. Reviewed goals, updated PSP and frequency of services, and discussed transition to Part B; direct work. Facilitated a nine month review with a family. All goals have been met, and the parents and PSP no longer have concerns about the childs development; therefore, we are discontinuing services; direct work. Completed an intake visit with a family; completed four screenings, received parental consent for evaluation, and discuss strengths, needs and daily routines of the family; direct work.
