Heather Pappas Tudor February 4, 2014 Personal Care ISTAR Assessment ISTAR KR: Oral Motor Rating: 3 and

4 On February 4th at approximately 11:15AM Tudor reached for his cut up peaches. He placed the peaches in his mouth using one hand. He then manipulate them in his mouth by using a chewing movement. After he swallowed the peaches Tudor grasped a piece of edamame. As he placed the edamame in his mouth he stuck out his tongue then manipulated it in his mouth as he had previously done with the peaches. Next Step: Displaying increased control of the rotary chewing movement and consistently keeping the lips closed while the chewing movement is performed.

ISTAR KR: Self feeding Rating: 2 During lunch on February 4th Tudor picked up his sippy cup filled with soy milk and tilted it in a slight upward motion. He then swallowed the soy milk and placed the sippy cup back out in front of himself. When he ate his small pieces of food he swung his arms back and forth which in turn made his sippy cup fall to the floor several times. Next Step: For Tudor to drink from an open cup and use utensils to feed himself.

ISTAR KR: Dressing and Undressing Rate :1 After Tudor was changed on February 4th at approximately 9: 45 AM I dressed him. I placed his feet through the leg portion of the garment. While I guided his foot through the garment he stayed on his back and made little to no movement. I finally fastened his buttons while he placed his arms at he side of his body. Next Step: For Tudor to assist in the removal/ or addition of clothing.

ISTAR KR: Care of hands, face and nose Rate:1 On February 4th at approximately 9:50 AM Tudor was assisted while washing his hands after his diaper was changed. As I turned the faucet on he reached his hands out towards the running water. I then retrieved his soap and scrubbed his hands together while I sang the ABCs. He kept his hand out towards the sink for the duration of the wash. Next Step: Reaching for the soap and rubbing his hands together without assistance during the washing of his hands.

ISTAR KR: Toileting Rate: No evidence Tudor arrived at the Child Study Center at 7:45AM. He was changed twice during the first session of the day that ended at 1:30 PM. Before his second diaper he cried for a few seconds. During both diaper changes Tudor remained mostly still with his arm at his side. His eyes looked up towards the mirror above him. Next Step: For Tudor to recognize when he needs a diaper change and effectively communicate that in some form.
