ACCIDENT WAIVER, ASSUMPTION OF RISK, AND

RELEASE OF LIABILITY

On behalf of myself, my spouse, my children, my parents, my heirs, assigns, personal
representative, and estate, I HEREBY ASSUME ALL OF THE RISKS OF USING
AND/OR PARTICIPATING IN ACTIVITY USING EXERCISE EQUIPMENT FROM
SLEPPSOLUTIONS, LLC, INCLUDING BUT NOT LIMITED TO THE HANDS FREE
UPPER BODY EXERCISE EQUIPMENT ("SleppSolutions Equipment"), including by
way of example and not limitation, any risks that may arise from negligence or
carelessness on the part of the persons or entities being released, from dangerous or
defective equipment provided or maintained by them, or because of their possible
liability without fault.

ABILITY TO PARTICIPATE
I certify that I have not been advised by a qualified medical professional to not
participate in physical activity or activity using the SleppSolutions Equipment. I
acknowledge that I have voluntarily elected to use and/or participate in activity using the
SleppSolutions Equipment for my own well-being.

VOLUNTARY PARTICIPATION
I expressly agree to accept and assume all of the risks, inherent, known or unknown, to
exist in this activity. My use and/or participation in activity using the SleppSolutions
Equipment is purely voluntary, and I elect to use the equipment and participate in activity
with the equipment in spite of any risks.

INHERENT RISKS OF PHYSICAL ACTIVITY
I acknowledge that use and/or participation in activity using the SleppSolutions
Equipment may result in physical or emotional injury, paralysis, death, or damage to me,
property, or third parties. I understand that such risks are inherent in physical fitness
activities and simply cannot be eliminated without diminishing the essential qualities of
the activity nor can those risks be specifically listed herein.

DISCHARGE OF LIABILITY
I WAIVE, RELEASE, HOLD HARMLESS, AND FOREVER DISCHARGE from any
and all liability, SleppSolutions, LLC and its representatives from any and all claims,
demands, or causes of action, which are in any way connected to my use and/or
participation in activity using the SleppSolutions Equipment, or the use by an individual I
invite to use and/or participate in activity using the SleppSolutions Equipment, including
any and all claims which allege negligent acts or omissions by SleppSolutions, LLC and
its representatives.

INDEMNIFICATION FOR FEES AND COSTS
Should SleppSolutions, LLC, its representatives, or anyone acting on their behalf be
require to incur attorneys costs and fees to enforce this agreement, I agree to indemnify
and hold them harmless for any and all such fees and costs.

INSURANCE AND/OR ASSUMPTION OF RISK
I certify that I have adequate insurance to cover any injury or damage I may sustain or
suffer while or in connection with using and/or participating in activity using the
SleppSolutions Equipment, or I agree to personally bear the costs of any and all injuries
or damages to myself, to others, and to damage or loss of property. I further certify that I
assume the risks of any medical or physical condition I may have prior to or concurrent
with my use and/or participation in activity using the SleppSolutions Equipment.

BROAD INTERPRETATION
I agree that if any portion of this agreement is found to be void or unenforceable, the
remainder of the document and remaining clauses shall remain in full force and full
effect. The accident waiver, assumption of risk, and release of liability shall be construed
broadly to provide a release and waiver to the maximum extent permissible under
applicable law.

JURISDICTION AND VENUE
In the event that I file a lawsuit against SleppSolutions, LLC or its representatives arising
from my use and/or participation in activity using the SleppSolutions Equipment, I agree
that venue shall be in St. Petersburg, in Pinellas County, in the state of Florida. I further
agree that the substantive law of the state of Florida shall apply to any action.

ACKNOWLEDGEMENT
By signing this document, I acknowledge that if anyone is hurt, including myself, or
property is damaged, resulting from my use and/or participation in activity using the
SleppSolutions Equipment, I may be found by a court of law to have waived any and all
of my rights to maintain a legal action against SleppSolutions, LLC, its representatives,
or those acting on its behalf.

I have had sufficient opportunity to read this entire document. I have read and
understood it, and I agree to be bound by its terms.
