REQUIREMENTS:

1. PESFA Accomplished Application Form

2. High School Report card with a passing grade with at least 80%
3. Birth Certificate
4. Income Tax return of Parents with annual gross income not exceeding
P120,000.00 in case of exemption, original copy of the certificate from the BIR or
affidavit under oath, certifying that the income of parents falls below the taxable
amount;
5. For orphans, a death certificate of parents should be presented while an
Affidavit of Support should be presented for those with guardians;
6. Certificate of Good Moral Character as certified by the school principal;
7. Recommendation from the Barangay captain;
8. Two (2) 2” x 2” photos take not more than six months before the filling of
application.

The deadline of filing of application is March 09, 2002(First Batch) and March
29, 2002 (Second Batch).
