Case : Mr. A, a 61-year-old man with hypertension, and symptoms of benign prostatic hyperplasia (BPH presents to his new primary care physician for a follow-!p e"al!ation. Physical e#amination re"eals inade$!ately controlled hypertension (1%&'() mmhg on hydrochlorothia*ide'triamterene with e"idence of ophthalmic and cardiac target organ damage. +he patient is also ta,ing se"eral medications that may interfere with his blood press!re control. He is also hyperglycemic (-B. 1/0 mg'dl, which may be partially attrib!table to the thia*ide di!retic. Patient ed!cation on life-style modification is the first inter"ention that sho!ld be !nderta,en. A change in dr!g therapy is also warranted, b!t the !s!al first- line medications, di!retics and -bloc,ers, may not be optimal for him. An antihypertensi"e that wo!ld also treat his BPH (i.e.an 1 antagonist is a reasonable choice for him. +he patient reports increasing !rinary hesitancy o"er the past 6 month, straining to !rinate, and fre$!ent noct!ria witho!t dys!ria, his prostate is enlarged on physical e#am. +his patient is abo!t 11 ,g o"er his ideal body weight and has a BM2 of 0/.&, he no longer smo,es anymore.
