I hope you have enjoyed time with your student-athlete during the summer break. However, in just a few
weeks, fall sports will be jumping into their first practices and student athletes will need to ensure they
have a completed physical on file.
s most of you are aware, we were not able to host sports physicals at !orthport during the "#$%-"#$&
school year due to changes in the e'amination process. (his in mind, there are two options to ensure
your student-athlete is prepared for the first day of practice.

a child wellness e'am to your insurance. If you have complications with your insurance, )r.

Complete physicals will need to be turned into the office before a student is able to practice. Coaches
will not be accepting documentation on site. If a student-athlete has not submitted a completed physical
by the first day of practice, they must be withheld per /H0 guidelines.
