Mr. S, aged 75years with the complaints o impairment o
intellectual unctions, impairment o memory was
diagnosed with Dementia !y a Neuro-Physician. Mr.Swas
was apparently normal or the past " years !ut started to
deteriorate in his cognitive unctions slowly. Now he is
ta#ing anti psychotic drugs, anti-dia!etic drugs and anti-
hypertensive drugs.
Ater an initial assessment, $ndia %ome %ealth &are's
Psychiatric Nurse has !een training the patient thrice a
wee# or the past one year. %e ta#es care o the
ollowing(
Medical &hec# )ist( *nce a wee# the nurse will chec# the
availa!ility o medicines, !lood pressure, !lood sugar and
ma#es sure that Mr.Sta#es the medicines promptly with
the help o his wie.
Saety chec# list( +o loc# the ront gate, to wear id card all
the time and ma#e sure his wie reminds him to use the
toilet periodically.
,ood chec# list( +o remind Mr. S. to chew the ood and eat
slowly, to have dry salted lemon pic#le -narthangai. along
with ood to maintain sodium level, to have Nutival
coconut oil orally everyday and turmeric supplement in
the night time.
/0ercise Program( &ognitive e0ercises and physical
e0ercises are done with Mr. S.

5. Photo reminder practice 3 +o identiy and relate their
relatives properly
6. Simple arithmetic calculations 3 Additions and

7. 8eading newspaper 3 %ighlighting day to day headline
news

training
9. &ycling e0ercises 3 Stationary !i#e training or 15
minutes thrice a wee#
7. Active and Passive e0ercises or the e0tremities in
standing position
Mr. S is leading a normal sae lie with a!ove mentioned
nursing help, medication and amily support. 2ith proper
caregiver education and monitoring !y a :uali;ed nursing
proessional at home, dementia patients can lead a
comorta!le lie with the support o a visiting nurse.
