AFFIDAVIT OF NO EARNINGS
(from self-employment as driver)
I, RESTITUTO CEREZO CANAVE, legal age, married, Filipino citizen, with
residence and postal address at No. 57 Kitma Rd., Bakakeng Central, Baguio City after
having been duly sworn to in accordance with law hereby depose and state that:
1. I am a driver of a motor vehicle and was employed by different companies or
corporations and private vehicle operators;
2. However, since I am suffering from asthma and now that I am old my
asthmatic triggered again, I stopped from my work as a driver last July, 2012.
As a consequence I have no more earnings or income;
3. I am executing this Affidavit to attest to the veracity and truthfulness of my
foregoing statement and to support my Retirement Claim Application at the
Social Security System;
4. Further executing this affidavit for all legal intents and purposes it may serve.
IN WITNESS WHEREOF, We have hereunto set our hand this 9TH day of August
2012 at Baguio City, Philippines.
