The 4 Mat Lesson Plan Format: for ELED 508

1. Objectives List your content objectives for this one lesson. (i.e. students will collect
demographic information on a sample of students). At this point no reference to the Common
Core is necessary. However, in the formal LP format, this is included.and should be with its
adoption.
2. Learning Activities List and describe in brief statements the activities that the students will
participate in. One does not have to script or break these into guided practice or independent
practice, just describe what is to happen.

3. Assessment Describe how the teacher will monitor achievement either as a pre assessment,
ongoing instructional assessment, or post assessment. The assessment can be as simple as
teacher observation all the way to formal objective or subjective tests.
