Resident B Assessment

Resident is a 17 year old male being admitted to the Olympus unit at Cottonwood Treatment
Center. He was willing to do the assessment, but appeared to be in a hurry and throughout the
assessment. He also appeared to have difficulty sitting in one position. His affect was cheerful;
he smiled often. Resident identified the behavior that led him to Cottonwood is
stealing. Resident also identified problems with: concentration, aggression, drug use, talking
about feelings, family relationships, seeing long distances, and sitting still. Resident indicates
that while at Cottonwood he would like to work on communication, aggression, and expressing
emotions.
Resident did not know the meaning of leisure. Intern informed him of the definition of
leisure. Resident states that leisure helps to calm me down. Resident reported enjoying listening
to music, watching TV, and going on walks in his free time. When Resident returns home, he
expressed a desire to join a soccer team. Resident identified that his creative ability is to figure
things out. Resident expressed limitations to his recreation and leisure participation includes:
money, does not know where, and homework/school. Resident states that walking around and
going to the park are activities he enjoys doing with his friends. Resident expresses his
relationships with his parent as pretty good. Resident reports that he considers himself a leader
because I like helping people out. Resident indicates that he cooperates well with a group of
peers because he tries to turn a negative into a positive when peers become upset. Resident also
reports that he cooperates well with adults. Resident stated that he expresses his anger and
sadness through isolation; and his happiness by talking a lot and being loud.
Current Physical Limitations, Precautions, and/or Special Needs: Resident did not report any
health problems at this time.
Therapeutic Recreation Recommendations:
Participation in structured therapeutic recreation activities 2 times per week to improve
functioning in the following areas:
Social Skills
Problem Solving
Self Confidence
Leadership
Teamwork
Goal Setting
Leisure Ed.
Decision Making
Coping Skills
Self-Awareness
Communication Skills
Therapeutic Recreation goals are documented in the residents Master Treatment Plan.

Short Term Goal:

During Recreation Therapy sessions, resident will use social


skills, i.e. self-advocating, assertiveness, positive thinking, and
emotional expression which will allow the resident to grasp a
wider view of his emotional experience. Resident will initiate the
sharing of insights from participating in problem solving sessions.
Resident will require no more than two prompts from staff during
recreation therapy sessions.
