Assessment Screenings

High blood pressure


Diabetic
Family history of kidney failure
Age 60 or older
Obese
African American, Hispanic,
Asian, Pacific Islander, or
American Indian

If so, you or someone you know


may be at risk of developing kidney
disease. Attend a nearby KEEP
Healthy event to fill out a risk survey
and receive more information. Free
of charge, professionals will measure
your blood pressure and conduct
blood and urine tests. NKFALL will
send you the test results and refer
you to health professionals as
needed.

Classic

Country Club
Show off your
golfing skills and hit the greens to
play 27 holes on secluded 310-acre
terrain. Known for its peaceful
atmosphere and scenic views, this
course is the perfect setting for a
golf tournament. Participants must
enter in teams of four, and winners
may compete nationally at Pebble
Beach, California in January 2016.
