Mr Rider was at the Stepahnie location for 4 hours/ week. From the outset, Johnny was very professional in his
demeanor and approach with the clinical staff and the patients. He demonstrates great empathy for the patients
and was able to relate to them at their level helping them to feel at ease with his treatment. In the 12 weeks he was
here, he has made tremendous progress with his clinical knowledge of the UE, application of modalities and his
manual skills. He is now able to assess and develop treatment plans for basic hand conditions such as carpal
tunnel syndrome , wrist fractures, etc. At this time, he is able to handle his own patient caseload (averaging
1-2/hour) and deliver appropriate treatments. His documentation for evaluations as well as daily notes show his
increased depth in clinical knowledge.

It has been a true pleasure having Johnny as my student this quarter. He is insiteful, competent, knowledgable
and compasionate. He takes his time with all patients to deliver high quality health care. His documentation is good
expressing patient response and advancement. He will be an asset to anyones program in the area he chooses to
practice.

Johnny has exceptional standards but needs to be exposed to a variety of situations to test safety skills. He is very
mature and a pleasure to have in the clinic. Overall his standards in the clinic have been very good.

Johnny has been working with several patients on a consistant basis and clearly expresses the goals and
expectations in therapy. He is so nice that he needs to be a little more forceful and direct with some patients. But
overall is performing as expected.

Mr Rider developed a mastery of our computerized documentation system in a short amount of time. He also
demonstrated competence in his goniometric assessments, use of Jamar dynamometer and pinch gauges. He was
exposed to various provocative tests in our discussions but I did not have the chance to observe him perform them
during an evaluation. He could use some improvement in incorporating more work related / occupation-based
assessments for our work comp population.

Nothing can replace practice and experience but he is currently functioning as expected. With continues
experience he will develop speed in completion of notes. His notes however are pertinent and appropriate using
correct techniques and tools

He was able to complete all his notes in a timely manner. He went above and beyond when completing his
assignments oftentimes gathering more information than was required. By the end of his rotation, Mr. Rider was
capable of seeing 1-2 patients/ hour . (Please note: We did not have any COTA nor aides at our facilities so these
particular skill sets were not observed during his rotation.)

Johnny is able to work independently using good judgement and clinical rational. We did not have assistance but
he had no difficulty interacting with other students in the clinic and I do not believe that Johnny will not have any
difficulty working and deligating to assistances.

Mr Rider is extremely professional and mature in his ability to communicate and relate to the patients. He has a
quiet but dignified way about him. He imparts confidence and maturity when dealing with patients, families and staff
members.

He works hard at understanding rational for therapy and appropriately discuss and educates patients, care givers
and family in home programs and tx rationales/diagonsis. Excellent inservice on Chemotherapy and Peripheral

It has been very rewarding working with Mr Rider because of his enthusiasm and passion for the profession and
hand therapy in particular. He always came in prepared for the week's assignment and went beyond what was
asked of him. He had very clear ideas of his goals which he communicated to me so that we could work on them
during his rotation, eg, increasing competencies in wound care and manual therapy. He asked intelligent and
thought- provoking questions, not just to challenge me, but because he is genuinely interested in improving himself
as a therapist.

Mr Rider's documentation skills for daily notes and re-evaluations are clinically relevant and appropriate. He was
not reliant on canned phrases from Therapy Source. Rather, he was able to articulate the patients' responses to
the treatments and has good ideas of how to progress their treatment plans when goals have been met. Since our
clinic adheres to the Biomechanical FOR, our treatments tend to be very exercise-based. Mr. Rider would be
encouraged to try and incorporate more functional activities into his treatments to make them more client- centered
