These questions are all optional, but the more material you give your recommender, the better your letter will introduce you as a unique individual. Type your answers to the following questions and staple them to this sheet, and fill out the reverse side of this form. * List three (3) words you would use to describe yourself, and explain each with a few sentences. * Is there anything unusual about your family or living situation?
