Petitioner is of legal age, Filipino, married, 34 years of age and a

resident of the Philippines, born on September 05, 1980 in Catarman
Northern Samar with a telephone number 055-500-6754 and with
office address at Barangay Daganas, Catarman, Northern Samar,
Philippines; whereas Respondent Civil Registrar are public officers
in-charge of the registration and cancellation/correction of entries in
the civil registrar where the fact of birth of the said petitioner was
registered, which registrar is located at Ground Floor, Municipal Hall,
Municipality of Catarman;
