Purpose:
Directions:
1. Use this form to develop an action plan for each Action identified through the SHI Module assessment process.
2. Be sure to save this form electronically and drop a copy into the SHI Materials drop-box on our course D2L site.
3. Be sure that each member has a copy of the plan and each time you meet to discuss the work on this portion of the SHI Group Project a
new copy of this document is provided electronically to all group members and updated in the D2L drop-box.
Goal:
Our goal is to have an after school program that allows students to stay after school and use the physical activity facilities. One way to encourage students
and staff to use the facilities would be to purchase new equipment for all to use. After 5:00p.m., the facilities would then be open for the community to use.
There would be a cost for community members using the facility; this cost would be 30 dollars per person per month or 50 dollars per family per month.
Staff of the school district would also have to pay a charge of 15 dollars per person per month or pay the 50 dollars for a family. The money earned
through this program will go towards paying staff members, new equipment, cleaning supplies, and healthy snacks. Staff working the after school program
will be paid 10 dollars an hour for their hours worked. Implementing the after school program will improve the districts wellness policy because it will
encourage all staff members to use the facility too. To ensure that safety of our staff and students, new and improved safety regulations will be applied.
Results/Accomplishments:
The result of having the physical facilities open to students and the public after school would provide a more affordable place for students and the
community to work out and be physically active. Also it would provide students a place to go after school if they do not have anywhere else to go.

By restating the steps and getting positive results/feedback, meeting deadlines listed in plan, gaining support from research that we gave the staff and administration
Benchmarks completing each step within the plan
Evaluation Process (How will you determine that your goal has been reached? What are your measures?)
