How comfortable were you in developing a professional relationship with your client and family. Provide an example of how you
used therapeutic communication this week.
I was comfortable developing a professional relationship with my client. I provided therapeutic communication for my client by
convincing her to ambulate, when she verbalized frustration over care provided, and grief over deceased brother. Patient was able
to express self and stated, I just gotta let it out.

Describe one assessment skill you did performed well and another you need to improve. Include your plan for improvement.
On my first day of care of my client I initially had some trouble hearing her breath sounds. With some guidance and tuning out the
noise of the construction I was able to pick up on the mild wheezing sounds. On my second day I assessed her breath sounds
much better and picked up on the mild wheezing during expiration in the anterior and posterior. To continue to improve on this I
just need to tune out other sounds and continue to listen breathing sounds.

Describe a new skill you implemented this week and your performance in completing it.
I was able to give an IV medication this week. I think I did well this time compared to the first time I did it. I looked up my
compatibility of my IV medications and found that it was cautioned, although I noticed that the previous primary nurses had not
changed the secondary line or back flushed, I thought it would be best that I would change my secondary line when I started my
Zosyn IV medication. The medication infused with no signs or report of pain from the patient and no infiltration.

Identify time management strategies that were and were not effective.
Coordinating medication administration with my primary nurse and instructor was challenging this week. Due to circumstances
out of my control r/t the patient IV site being compromised, having an allergic reaction, my primary nurse on the second day
preferring to give the IV med to stay on time, my classmates also giving meds, and my primary nurse assisting her other
patients. I realize these things cannot be helped and one must be flexible so as I waited, I tried to assist my patient in other things
and my classmates.

Were you involved in using interdisciplinary resources for your client? What resources could your client have benefitted from?
My client would benefit from having a physical therapist and dietician. Due to her weight, osteoarthritis to her knee, and cellulitis
on RLE she has some mobility impairment. With a physical therapist they can help her in her mobility and a dietician can assist in
coming up with a meal plan to aid in weight loss.

Describe an actual or potential legal or ethical issue related to the care of the client.
A potential legal risk would be negligence/malpractice r/t mixing cautionary IV medications. It is important to check
compatibility of IV medications and appropriate measures should be taken such as changing the IV line or back flushing to
prevent any harmful reaction to the patient.

Identify a safety risk for your client (using the safety rubric) and describe how you reduced the clients risk for injury.
A safety risk for my client is IV medication toxicity by mixing cautionary meds. I observed the administration of the
Vancomycin by my primary nurse who was using the same line that infused Zosyn, the line was not back flushed or changed. Prior to
starting my IV med (Zosyn) I checked compatibility and found that there was a caution when infusing them. I decided to change my
secondary IV line to prevent any toxic reactions.
