One of the strengths related to supporting the EMR is to decrease errors and improve
data accuracy. According to Lee, Kuo & Goodwin, Numerous single site studies at
academic hospitals provide evidence that specific functions of the EMR, such as clinical
decision support or computerized physician order entry, may improve quality by reducing
errors ( 2013). It was also stated that, The Institute of Medicine (IOM) has encouraged
adopting EMR to reduce medical errors, and the American Recovery and Reinvestment Act
(ARRA) of 2009 established financial incentives for hospitals to promote the adoption and
meaningful use of health IT (Lee, Kuo, & Goodwin, 2013). Another benefit is it will
improve the record keeping process and will be more efficient. EMR will allow nurses to
retrieve information in more timely matter, which will support them with the critical
decision-making needed to provide safe patient care.
Interdisciplinary Team
An EMR system improves interdepartmental communication (Kumar & Aldrich,
2010, p.308). Using EMR is will decrease wasted time spent re-explaining symptoms to
each nurse, physician, and specialist, which can make the treatment plan much more
difficult. Also, this will ultimately speed up the correct plan of care for the patient and
reduce the chances of any complications.
Clinical information can be shared via electronic transactions with all entities within
the health information network (Yoder-Wise, 2014, p. 207). Which means multiple care
providers, in different locations, can simultaneously view a patient's medical record on their
computers and get up-to-the-minute information. This collaboration enables care providers
to work more efficiently to achieve the best possible outcome for each individual patient.
Outcomes
After analyzing the data, the team next sets a goal for improvement (Yoder-Wise,
2014, p. 399). The team members should include someone from all areas in which the EMR
will be utilized in order to meet the criteria to meet the standard of care. Yoder-Wise states,
Nursing-sensitive indicators reflect the structure, process, and outcomes of nursing care
(2014, p. 399). There are many outcomes that implementing EMR would be considered such
as improve quality, safety, and efficiency and reduce health disparities, engage patients and
families in patient care improve care coordination, ensure privacy and security of protected
health information (Sheridan, 2012). According to Holroyd-Leduc, Lorenz, Straus, Sykes,
& Quan, There are clear advantages over traditional paper-based records in terms of
legibility and accessibility (2011). This outcome will decrease the risks of errors occurring
and will allow more members of the interdisciplinary team to collaborate together to achieve
goals in all areas of the patients treatment.
In order to meet the goals of the possible outcomes, it is important for nurses to
embrace technology to improve nursing practice. Leadership will play an important part
during the groundwork in preparation of implementing an EMR system. The bottom line is,
To continue to provide patient-centered care in a technologic society that strives for
efficiency and cost-effectiveness (Yoder-Wise, 2014).
Implementation Strategies
Change is a challenging process and leaders must know how to navigate it
(Yoder-Wise, 2014, p. 326). There are steps that can be taken in order to have a successful
transition to go from paper documentation to the EMR. Step one, is to assess what the needs
and goals are for the practice. Also, evaluate what the budget is. Cost will effect what form
of EMR will be utilized. Second, choosing the right EMR system for your practice, this
step will take a lot of time to complete all the research needed in order to purchase the right
system for the practice. After the installation of the EMR system, the training process
begins.
Evaluation
According to Belletti, A survey conducted by the Medical Records Institute and
Professional Risk Associates from March to June 2007 showed that EMRs can help
promote teamwork across disciplines by enhancing the visibility of each disciplines work
(2010). The American Medical Association (AMA), are also pushing for the greater
adoption of EMR to foster improved treatment, public health, patient safety, quality
improvement, medical liability defense, and research. Some insurers are also offering
discounts on malpractice insurance for physicians who utilize EMR systems (Belletti,
2010).
One thing to consider when implementing the EMR is the time it will take for
training. This may decrease productivity while learning the new system. Documentation will
take extra time until it becomes more familiar. Overall, once the EMR becomes easier to
use, the documentation time will decrease, which will allow more time spent doing other
things such as time with the patients.
