Goals
Related to CNOs guideline 3.16, Identifies her or his accountability for
Quality Assurance (CNO Guideline, 2012).
1. I will demonstrate professionalism through making sure I arrive
to clinical on time, prepared with all the required items (name
tag, clean scrubs, watch, completed care cards and med profiles)
and maintaining healthy and appropriate relationships with the
residents and other staff members at Lakefield.
2. I will take professional responsibility for myself by knowing my
scope of practice as a nursing student and will only provide the
care that I can do safely and appropriately.
Activities, Necessary Resources and Timeframes to achieve my goal
I must remain aware of my expanding scope of practice as a
nursing student
Ask for help with tasks that I cannot do safely by myself
Obtain feedback from the PSWs and other people within the long
term care facility
Evaluation
How will I know that I have achieved my goal?
- I will know that I have achieved my goal of arriving prepared each
week when I arrive at clinical and I can evaluate if there are things I
must change to achieve this goal (ex. leaving earlier, preparing all the
things I need to bring the night before clinical etc.)
- I will know that I have achieved my goal of only providing care that I
am able and allowed to provide if and when I am asked to do
something that I am not comfortable with and I respectfully inform the
person asking (PSW, RN etc.) that I am unable to perform the task.

Registered Nurse practice (CNO Guideline, 2014).
1. I will practice client centered care by collaborating with residents
to develop care plans that address their medical needs as well as
personal ones (social, spiritual, mental etc.)
Activities, Necessary Resources and Timeframes to achieve my goal
Revisit client centred care lecture slides
Identify all major health risks for each resident and make sure
they are represented in the care plans for the individual

How will I know that I have achieved my goal?
- I will know that I have achieved my goal of providing client
centred care by helping residents make their own care plans
when a care plan has been made for the resident that I am
assigned to that she feels addresses all of her needs.

Registered Nurse practice (CNO Guideline, 2014).
1. I will promote a safe environment for residents, self and other
staff members at Lakefield Extendicare, while addressing the
different care needs of the individuals living there.
Activities, Necessary Resources and Timeframes to achieve my goal
Review the standards for when and how to don and doff PPE
Use proper hand hygiene
Ensure uniform is clean and free from contaminants from outside
the care home
Evaluation
How will I know that I have achieved my goal?
- I will know that I have achieved my goal of promoting a safe
environment when given the chance to preform the different
types of precautions (airborne, droplet and contact) by doing
them properly. Also, by washing my hands before entering
different rooms to provide care and when leaving them will bring
me towards achieving my goal.

Revisit residents chart to make sure that all of their medical
needs are being met
Ensure that all care delivered to the resident is done in a
respectful way

- I will know that I have achieved my goal of advocating for the
residents when the residents are being cared for in a way that
they are pleased with and they dont have any major concerns or
questions about their quality of care.

1. I will make sure that I only practice the skills on my resident that
I have been trained and am able to do. For example, blood
pressure and cardiovascular assessments.

Review lab material before going to clinical to ensure that the
tasks I am able to perform are done correctly
Practice the skills that I am able to do multiple times before
going to clinical (ex. practicing blood pressure and palpating
