I use informed practice when
administering medication to patients by
ensuring I follow the eight rights of
medication. This is considered best
nursing practice to ensure client safety. I
also used evidence based practice by
holding a blood pressure medication if it
is not in parameters and if no parameters,
hold if BP too low.

I demonstrate this when I ask a PSW to
assist me with a hoyer because if I wanted
to help my patient with another student
that would be considered unsafe due to

physiotherapists, when providing morning
care. Some of my patients have ADLs
with the OT assistants so I would start
morning care for the patients who do not
have ADLs. The PSWs provide help when
assistance is need to use the hoyer lift.

I held her hand to provide comfort which
is what I would like as it helps me feel

combing her hair because as an elder it is

I managed to prioritize my nursing care
when I was taking care of two patients.
One had an early physio appointment and
one had a later appointment. So the
patient that had the early appointment I
bathed and dressed first and my second
patient I helped afterwards.

My patient prefers to have mouth hygiene
before breakfast and also after breakfast
so I incorporated that into her care by
assuring that I brushed her teeth after
breakfast so my patient feels ready and
clean for daily appointments.

that are holistic and client centered and
are articulated competently in writing
and orally.
My patient was a 2 person assist and after
seeing much improvement the nurses
changed the patients ambulatory status to
1 person assist which was immediately
recorded in the kardex.

nails cut but nurses did not want to risk

when a new medication needed to be
ordered for a client. I helped a nurses

briefs for a patient so that my partner and
I can assist the client in rotating to the
sides the bed when I am providing

information for the development of NCP
Step 1 and 2. I also use my pharmacology
text to get information on the drug cards I
make and lastly I google peer reviewed
sites to obtain information on medical
terminology

I wear appropriate footwear that requires
full leather coverage instead of some
running shoes that have a net coverage.
This provides a barrier to protect from
contaminated fluids. This has been
monitored at this hospital in light of the

entering a room of a patient with C-diff. I
keep the doors closed to the patients
room and anteroom, which happen to be
propped up at the beginning of shift, and
all PPE that maintains precautions for

after removing gloves and apply PPE
