I. Background:
Rxxx lives with his parents and older brother. He currently attends xxxxxxx
Elementary school and is in the second grade. According to his mother, Rxxx is not
currently receiving any additional speech language therapy services. Rxxx's mother
noticed clarity issues with his speech and contacted the Nazareth Speech and Hearing
Clinic after being referred by Speech Language Pathologist Edith Turner of the
Rochester City School District (RCSD). Previously Rxxxx attended the Nazareth
College Speech Language-Hearing Clinic in the spring semester 2013. He was evaluated at the clinic by
a graduate clinician, Megan Tice, who administered the Weber Spontaneous-R Probe and received a
baseline score of 0%. The focus of therapy last semester was on initial /r/ (right) and discrimination of a
clear and unclear /r/. Phonetic placement cues, the use of mirrors, and a positive behavior chart were
used in therapy last semester to help facilitate correct production of the /r/, however Rxxxx did not meet
any of his clinical goals and objectives.
II. Present Level of Performance
Rxxxx was very energetic and willing to begin therapy. During the first session baseline was taken by
administering the Weber Spontaneous-R probe-words resulting in 11% accuracy in multi-position /r/
words initial (right), medial (dirt), final (spider), and ire (tire). Rxxxx was observed to have more success
with ire getting 50% of those words correct. Rxxxx was able to approximate the /r/ in the initial position of
words when using a mirror to visualize his /r/ and remind him of correct tongue placement and to not to
place his upper teeth on his lower lip. Rxxxx is aware of his clarity issue and is able to discriminate
between an error sound and a correct production.

Rationale: Children will often substitute a "w" or "y" sound for the difficult to produce /r/ sound. According
to Shipley,K.G. and McAfee.J.G., this process should begin to fade and a correct production of the /r/
sound should emerge around the age of 5:0 (1998). Focusing on improving the production of the /r/
phoneme will improve Rxxxx's speech clarity. Improved speech clarity will improve the ability of others to
understand Rxxxx clearly. Improving his speech clarity will also build confidence since he is aware of
clarity issues. Correction and improved clarity of the /r/ is also functional for Rxxxx seeing that his name
begins with this sound.
