9. School records showing the name, age and residence of the adoptee;
10. Affidavit of Consent to the Adoption by the:
a. The adoptee, if ten (10) years of age or over
b. The biological parents of the child, if known, or the legal guardian, or the
child-placement agency, child-caring agency, or the proper government
instrumentality which has legal custody of the child;
c. The legitimate and adopted children of the adopter and of the adoptee, if
any, who are ten (10) years of age or over;
d. The illegitimate children of the adopter living with him who are ten (10)
years of age or over; and
e. The spouse, if any, of the adopter or adoptee.
11. Clearance from the following government agencies for both petitioners
a. NBI
b. Police Clearance
c. City Prosecutors Office

d. MTC Office of the Clerk of Court


e. RTC Office of the Clerk of Court

12. Proof of Financial Capacity


a. Latest income tax return
b. Bank Statement
c. Certificate of Employment
d. Proof of Business ownership if any
e. Proof of ownership of real or personal property i.e. land, bldg., motor
vehicle
f. Statement of Assets and Liabilities (if government employee)

13. Medical Certificate of petitioners issued by a doctor in a government hospital.

14. Affidavit from three (3) character references, - a non-relative member of the
immediate community who have known the applicant(s) for at least three (3)
years, attesting the good moral character of petitioners;

15. Pictures of the applicant(s) and his/her immediate family taken within the last
three (3) months;

16. Certificate of Attendance to pre-adoption fora or seminars.

17. Certificate of Residence from the Barangay.

For the alien petitioner, Certificate of Residence in the Philippines issued by the
Bureau of Immigration or Department of Foreign Affairs, as appropriate;

18. Certification by his diplomatic or consular office or any appropriate government


agency that he has the legal capacity to adopt in his country and that his
government allows the adoptee to enter his country as his own adopted child.
