Provide a description of the project including key strategies, timeline for project.
The particular project we are searching funding for is the fight against cardiovascular disease in
women for the middle to upper age class. We have deemed the project title as stated; "Women
Against CVD." Currently, our best plan of action is to create an active learning environment
along with extended sessions of teaching the community to control their cardiovascular diseases
or inherent risks. We plan to have proper health related professions run each facet of the
program. This will be a 7 week program to help jumpstart the community into hopefully
continuing the healthy habits learned.Also, the first two days of every phase will be directed
towards education on that subject matter. The first two weeks will be geared towards establishing
healthy fitness activities such as walking, light jogging, and water activities. Essentially ways to
improve cardiovascular function and reduce possibilities of weight gain is the key in this phase.
Phase two, consisting of two weeks will geared more on the subject of proper nutrition.
Participants will be educated on proper nutrition and will contain a vast majority of cooking
workshops. Pharmaceuticals will also be discussed within this phase. Phase three will also be 2
weeks, and will consist of team building exercises to focus on the mental fortitude when fighting
cardiovascular disease. Team building and communication allows the class to integrate into
teams of 5 to 7, and work together for the common goal of fighting this deadly disease.
Counselling and open speaking groups will be extended to the group. The final week will be 2
months after the sixth week. This will be the final assessment to see if the participants were
capable of fighting cardiovascular problems after, but in the absence of professional guidance.
3. Describe the need that the program will address Needs Statement
The sole purpose of this program will be to address the risks of CVD. With proper guidance,
community programs will allow each affected individual woman to have a stable chance of
maintaining or reducing the probability of a cardiovascular issue.

6. List the strategies you will utilize to reach the program objectives- Methods
1. Send out flyers, and publicly display advertisements (posters) around the city of Boone to
build the amount of participants.
2. Recruit program coordinators, volunteers, and other health experts (6-10).
3. Create the budget.
4. Plan fitness, nutrition, and mental health program with recommendations.
5. Print informational packets that detail the entire program for participants.
6. Have an orientation day where the program coordinators will introduce themselves and
purpose of the project. Participants will be advised to freely ask as many questions as
possible.
7. Participants will be given a date to pay the program fee of $25.00.
8. Make necessary purchases for the programs needs (mats, stop watches, water, videos,
etc).
9. Administer survey for pre-program period. Needed for compilation of data and future
assessments.
10. Participants begin the program, meeting every 3 days as required.

Watauga Healthy Carolinians and the participants of Women Against CVD will evaluate the
overall effectiveness of the program to help determine if the objectives were met, or surpassed.
Evaluations will include:
Pre and post-surveys that will overview the health behaviors, practices, and interests of
participants before and after the program.
Documentation of the amount of participants that finished the program.
Questionnaires to assess the educational attainment of physical, nutritional, and mental
health.

After the program has been completed, a summative report will be compiled to assess the general
effectiveness of Woman Against CVD. Base on the evaluation of the report, the program will
begin to make necessary modifications to better the program if deemed desirable.
8. Describe how your program will be continued after the funding period. Specify source
of funding and commitment of your agency Sustainability Strategies
Women Against CVD will continue to follow up with the participants and work closely to search
for potential funders. Each participant is welcome to complete the course again, as any health
information will be updated for each yearly program. For each possible participant they bring
with them, a deduction of $5.00 will be subtracted from the rate of $25.00.
