Photography & Videography Release form

Elementary School. (S)he is a student in Miss B's Class. I Understand
that there will be Thematic Units and or class/school events that will
involve photography and/or videos of my child. It is to my
understanding that these photos will not be released outside the
classroom or preschool's boundaries.

Example of class Unit using Photos:

Miss B's Class will soon be working on a self-esteem unit. This
unit will teach them to identify positive and negative words,
how to self-motivate, and much more!
One of the crafts projects will require a printed head shot with

paper posted to the back. That lined paper will be filled out
by the student (with assistance) saying Why I love Myself.
It is very important to develop self-esteem and respect for
other at a young age. These are lesson you will want them to
learn now and hold on to for years to come!
