INCREASE IN SUM ASSURED POLICY # 603210905-0

Sir,
I bought above policy in May, 2008 for sum assured of Rs. 500,000 with ADB rider
under Table 03 Term 20 years.
It is requested that sum assured may please be increased from 500,000 to
2,000,000 from the same date i.e. 21-5-2008 with same term and conditions.
I hereby submit my original policy documents, last premium paid receipt and
Personal Statement of Health.
