Transition Checklist

o Make contact with feeder schools by May 15 in regards to any incoming

students with academic or behavior needs
o Set a date with high school transition team to meet by May 15
o Complete online transition forms for grade 8 students with special
medical, behavior or academic needs
o Review incoming transition information with grade level teams and
provide copies of information at June Team Meeting
o Send copies of Policy 704 for grade 8 students with medical needs to
appropriate High School by June 15
o Document transition planning in PLP of grade 8 students moving on to
high school
o

Update PLPs of grade 8 students to reflect any new documentation (ie:

psycho-educational assessment, O.T. assessment etc.) by the year end
