    Sir/Madame:
    Respectfully requesting for replacement of RCBC Check No. 1200000497 dated
    September 24, 2014 in the amount of Three Thousand Nine Hundred Thirty Five and
    28/100 (P3,935.28) which was issued to my late husband Antonio C. Rich and was
    received by the undersigned on February 25, 2015. My husband died last October 5,
    2014 after 6 years of illness.
    ChinaBank Maasin Branch refused to accept the check for deposit as the payee is my
    late husband. I was advised instead to return it back to the issuing bank. The check is
    now stale since six months has already lapsed since date of issuance. I was not able to
    process it due to the demands of my work.
    I hope you will consider my request for replacement, as the amount will be of great help
    financially.
    Attached are pertinent documents to support my request.
