Transition meeting checklist:

Why:

plan for students changing schools or starting school


meet with key people involved in the transition process
discuss student support needs
clarify what needs to happen for successful transition

Setting up and inviting:

arrange a mutual meeting space and book the space if needed


organize a support person to take notes
potential invitations to: parents, early childhood intervention, early childhood teacher,
principal, classroom teachers, resource teachers, and school counselors
