AFFIDAVIT OF DISCLOSURE OF NO RELATIONSHIP

I, FRANCO GRAYDA MADRID, of legal age, Filipino, married and
residing at Zone 1, Bulan, Sorsogon, after being sworn in accordance with law, do
hereby depose and say:
1. That I am filing my certificate of candidacy as Board Member in SORECO
-I, with the office address at Gulang Gulang, Irosin Sorsogon;
2. That I have no relationship within the fourth civil degree of consanguinity
or affinity to any member of the EC Board of Directors, Department
Managers, NEA appointed Project Supervisor (PS) or Acting General
Manager (AGM)and its equivalent or higher positions ( Sec. 15 (f), IRR);
3. That I execute this document to attest to the foregoing facts and for any
legal purposes this may serve.
